The best text-to-speech app we've used is Dragon Dictation. Dragon Dictation for the iPad is a free app that turns your spoken words into text. It’s fast, efficient, and accurate. Just tap the little Record button, then start talking. Tap again when you’re done and the app quickly returns your translated text. Another tap and your text becomes an e-mail, a text message, or a Twitter/Facebook post. It's supposedly 5 times faster than typing on the keyboard. It works best in a quiet environment and you'll need to be connected to the internet since it does real-time translation over the internet.

QuickVoice2Text Email is another app that can translate voice into text, which you can then email the actual audio recording, email the audio + text, or save as a ringtone. The downside is that you don't get to see the transcription before it's sent--that happens on their servers--so you can't edit typos--so it's best to use for non-business emails.
The video is a demo on the iPhone but it works on the iPad too.
